Division Director of Case Management

The Division Director of Case Management is responsible for planning, managing, directing and evaluating the activities, functions and personnel of the Case Management Departments in designated hospital facilities.  Contributes to the development and execution of the strategy to integrate the operations within the hospital system and standardize practice of case management in assigned hospitals.  Leads and directs all activities related to the provisions of Case Management, Utilization Management, Social Work, and Clinical Documentation services as assigned. Coaches and develops members of the case management team and cultivates an environment of innovation and improvement to distinguish the department as “best practice.” Develops and implements an infrastructure necessary for a robust, data driven, and outcome oriented case management service. Collaborates with hospital leadership to establish outcome targets and identify and resolve barriers to reaching targets.

Requirements:

·         Must have current Illinois licensure to practice professional nursing

·         Master's Degree is required.  (Will consider qualified candidate if actively pursuing graduate level education.)

·         Minimum five years management experience. Minimum 3 years Case Management experience.

·         Must be certified as Case Manager from approved certification program: ACMA or CMSA

·         Must possess listening and communication skills (both oral and written), decision making and problem solving techniques and the ability to handle stressful situations.

·         Must keep abreast of new case management and other healthcare trends and make effective recommendations for implementation of innovations as necessary.

·         Possesses effective leadership skills.

·         Is knowledgeable of state and federal laws and hospital and divisional/departmental policies, procedures and directives.
